Registration Information and Forms


    
     

A Centralized Registration for all South Central School students will be held on Tuesday, August 11th from 11:00 a.m. to 7:00 p.m. and Wednesday, August 12th from 9:00 a.m. to 12 p.m.  All students will be registering at the Middle School building located at 503 S. Madison, in Kinmundy.  High school students must be accompanied by a parent or guardian in order to register. Proof of insurance is required for all sports, OKAW and shop classes. All forms needed for registration can be found below.

Students entering Pre-school, Kindergarten, 6th and 9th grades must submit a school health physical, up-to-date immunization (shot) record at registration. No student will be allowed to register without this information.  Dental Exams are also required byMay 2010 for Kindergarten, 2nd, and 6th grade students.

Required Documentation for New Pre-K and Kindergarten Students

At registration in August, you will need to provide the following:

  • A school physical completed after August 18, 2008 (last August).
  • Proof of updated immunizations, including Varicella (chicken pox) or proof of disease.
  • Proof of dental exam dated after November 15, 2007
  • A copy of your child's birth certificate.
  • Your child's Social Security number or copy of card.
  • Verification of Residency
  • Complete a lead questionnaire.

Registration will not be completed without the above information.

  PE Uniforms for 5-8 grades: $15.00
  PE Uniforms for 9-12 grades: $20.00
  Parking Permit for High School: $20.00
  Driver's Education: $40.00
